We are seeking a dynamic, empathetic, and dedicated Consulting Patient Navigator Specialist to join our growing team. The ideal candidate will have a background in outpatient clinic support, administrative healthcare work, and HIV/STD treatment and prevention with a focus on harm reduction. This role requires a minimum of 2+ years of experience in a similar role. You will be working directly with patients, guiding them through their healthcare journey, and providing the necessary support to ensure they receive the right care at the right time. Responsibilities: 1. Serve as the primary point of contact for patients, assisting them in navigating through the healthcare system and coordinating their care. 2. Provide education and counseling to patients about their diagnoses, treatment options, and available resources. 3. Collaborate with interdisciplinary teams to develop, implement, and evaluate patient care plans. 4. Facilitate communication between patients, their families, and healthcare providers to ensure patient needs are met and concerns are addressed. 5. Advocate for patients' rights and needs, ensuring they receive appropriate care and services. 6. Assist patients in understanding and managing their healthcare bills, insurance, and other financial matters. 7. Conduct outreach to at-risk populations, promoting HIV/STD prevention and treatment strategies, and harm reduction approaches. 8. Maintain accurate and timely documentation of patient interactions, interventions, and outcomes. 9. Participate in continuous learning and professional development activities to stay updated with the latest trends and best practices in patient navigation and healthcare. Qualifications: 1. A minimum of 2+ years of experience in patient navigation, healthcare administration, or a related field. 2. Experience in outpatient clinics, providing support to patients. 3. Knowledge of HIV/STD treatment and prevention strategies, with a focus on harm reduction. 4. Exceptional interpersonal and communication skills, with the ability to interact effectively with diverse patient populations and healthcare professionals. 5. Strong advocacy and problem-solving skills, with the ability to effectively coordinate care and resolve patient concerns. 6. Proficiency in health information management systems and Microsoft Office suite. 7. Ability to maintain patient confidentiality and adhere to ethical standards. 8. Bachelor's degree in healthcare administration, nursing, social work, or a related field is preferred. 9. Certification as a Patient Navigator or similar certification is a plus.
