Overview
Medical Social Worker - Social Services Practitioner III (SSP III) - The Social Services Practitioner III position plays a significant role in supporting patient care coordination and discharge planning. The SSP III assists medical staff in addressing clinical situations and complex cases related to children or adults and their families. Responsibilities include social determinants of health screenings, psychosocial assessments, crisis counseling, discharge planning, and linking to community resources and funding sources.
The Riverside University Health System Medical Center in Moreno Valley is seeking SSP III to work as a Medical Social Worker. Under general direction, the position performs difficult social service casework in healthcare facilities, plans, organizes, implements, and evaluates clinical services, and may participate in in-service training programs. The SSP III may provide leadership as a program consultant in casework concepts and methodology.
Location: Moreno Valley; Schedule: Sunday – Thursday or Tuesday – Saturday; 8-hour shift from 9:00 AM to 5:30 PM. The schedule may be adjusted based on departmental needs.

Examples Of Essential Duties
Carry a caseload of difficult social service cases; assess client problems, develop treatment plans, and lead self-care, well-being, and educational courses for community participants and/or staff.
Complete case studies to assess problems and determine appropriate treatment methods; develop intensive long- or short-term treatment plans.
Act in a lead capacity for a small group of Social Services Practitioners or service employees (aides and volunteers).
Act as a casework consultant to staff members without professional training.
Function at a highly skilled level in counseling, protective services, medical social work, family services, community organization, and adoptive services.
Participate in staff development programs to increase knowledge of social work processes and technical competence.
Provide therapeutic marital, family interrelationship and adoptive parent counseling.
Investigate and provide services to clients with issues such as neglect, abuse, emotional or behavioral problems, disabilities, or other health conditions affecting family dynamics.
Provide services and counseling in cases of desertion or impending loss of a spouse, child, or caregiver.
Interpret public social service rules, regulations, and policies to clients and the public.
Maintain casework records and prepare reports and correspondence.
Conduct field visits to carry out service plans, observe client needs, and provide social work services.
Assess family environments to determine risks to children/disabled adults/elderly; where safety requires, remove individuals from home and transport them, complying with safety regulations.
Maintain up-to-date electronic records of client case management in a centralized database.
In addition to the above duties, assignments within healthcare facilities may include:
Study patients and families to determine problems and develop effective treatment plans.
Interview patients, families, and friends to obtain information for case records.
Assess the psychosocial needs of pediatric patients and initiate appropriate interactions.
Coordinate and consult with medical staff and outside agencies regarding available resources.
Interpret significant emotional, social, and economic factors to the healthcare team.
Participate in seminars or training programs to enhance social work capabilities; may participate in group therapy.
Assist in training social service aspects of medical treatment for healthcare personnel outside the social services unit.
Under supervision, provide social casework services to help patients and families adjust to problems caused by illness or disability.
In addition to the above duties, assignments within the Child Life Program may include:
Plan, implement, and evaluate the Child Life Program.
Minimum Qualifications
Medical Social Worker (Social Services Practitioner III)
Education:
Master’s degree in Social Work from an accredited college or university.
Knowledge of:
Principles and techniques of social casework; development and family relationships; socioeconomic and cultural factors; laws and regulations governing public social services; basic computer skills.
Ability to:
Establish rapport with clients; apply casework knowledge; plan and analyze; work within an agency and community setting; utilize resources; provide consultation and supervision to less experienced staff; input and retrieve data from computerized systems.
Other Requirements:
License/Certificate:
Valid California Driver’s License may be required. BLS certification from the American Heart Association is required upon hire or within 6 months of hire.
