Medical Social Worker – Social Services Practitioner III, Moreno Valley
Master's degree in Social Work REQUIRED!
As a Social Services Practitioner III (SSP III) at RUHS Medical Center, you will play a key role in supporting patient care coordination, discharge planning, and addressing complex clinical situations involving patients, families, and the broader healthcare team. You will conduct social determinants of health screenings, psychosocial assessments, crisis counseling, and linkage to community resources and funding sources.
Responsibilities
Carry a caseload of complex social service cases requiring high technical competence.
Assess clients’ problems and develop treatment plans.
Provide lead self‑care, well‑being, and educational courses to community participants and/or staff.
Complete case studies to assess problems and determine treatment methods.
Develop intensive long‑ or short‑term treatment plans to improve or restore individual or family functioning.
Act in a lead capacity for a small group of Social Services Practitioners or support staff (aides, volunteers).
Serve as a casework consultant to staff members without professional training.
Function at a highly skilled level in counseling, protective services, medical social work, family services, community organization, and adoptive services.
Participate in staff development programs to increase knowledge of social work processes.
Provide therapeutic marital, family interrelationship and adoptive parent counseling.
Investigate and provide services for clients with physical or emotional welfare concerns, including neglect, abuse, or health‑related issues.
Provide services and counseling in cases of desertion or impending loss of spouse, child, or caretaker.
Interpret public social service rules, regulations and policies for clients and the public.
Maintain casework records and prepare reports and correspondence.
Frequently conduct field visits, including long distances, to carry out service plans and assess client needs.
Assess client family environment to determine potential or imminent risk to the safety of children, disabled adults, or the elderly.
Remove or facilitate placement changes when safety needs dictate, complying with all safety regulations.
Maintain up‑to‑date electronic records of all aspects of client case management in a centralized database.
Perform studies of patients and families to determine the nature of their problems and develop effective treatment methods.
Interview patients, families, and friends to obtain information for case records.
Assess psychosocial needs of pediatric patients and initiate appropriate interaction.
Coordinate and consult with medical and other staff members and with outside agencies regarding available resources.
Interpret significant emotional, social, and economic factors for the healthcare team.
Participate in seminars and training programs to enhance social work capabilities; may participate in group therapy.
Assist in conducting training of social service aspects of medical treatment for healthcare personnel outside the social services unit.
Under close supervision, provide social casework services to help patients and families adjust to social, emotional, and financial problems created by illness or disability.
Plan, implement, and evaluate the Child Life Program.
Qualifications
Education: Master’s degree in Social Work from an accredited institution.
Knowledge of principles and techniques of social casework, public social service agencies, individual and group behavior, family relationships, socioeconomic factors, mental illnesses, community organizations, laws and regulations governing public services, and basic computer skills.
Ability to establish rapport with clients and families, apply casework knowledge, evaluate personal and psychological factors, work constructively within an agency and community setting, utilize appropriate resources, provide consultation, demonstrate planning and analysis skills, control bias, maintain professional ethics, and assist in professional development of less experienced practitioners.
Other Requirements
Valid California Driver’s License (may be required).
Valid certification in Basic Life Support (BLS) from American Heart Association required upon hire or within 6 months.
