Conduct comprehensive psychosocial assessments and develop individualized treatment plans
Provide counseling, therapy (individual/group), and patient/family education
Reassess and update treatment plans based on patient progress
Deliver case management services and coordinate care across providers
Assess Social Determinants of Health and connect patients to resources
Collaborate with patients, families, and care teams on discharge planning
Maintain accurate and timely documentation in compliance with policies and regulations
Respond to behavioral health and medical emergencies as part of the care team
Ensure patient confidentiality in accordance with legal and regulatory standards
Refer patients to appropriate community agencies and support services
Tailor care based on patient population needs (age, culture, language, etc.)
