Patient Care Pharmacist, Inpatient Pharmacy

Job Responsibilities
  • Review medication orders for appropriateness (dose, route, frequency, indication, interactions, allergies)

  • Verify and dispense physician‐ordered medications in accordance with hospital protocols

  • Perform medication reconciliation upon patient admission, transfer, and discharge

  • Monitor patient therapy outcomes and adjust or recommend changes to optimize treatment

  • Provide drug information to healthcare providers, patients, and caregivers

  • Evaluate and prevent adverse drug events and medication errors

  • Ensure compliance with hospital formulary and therapeutic guidelines

  • Maintain inventory control of medications, including controlled substances

  • Document all activities in the electronic health record (Epic)

  • Ensure compliance with state and federal regulations

  • Precept pharmacy students and residents in clinical and operational settings

Position Qualifications

Minimum Required:

  • A Bachelor's of Science Degree (B.S.) or Pharm.D. in Pharmacy from an accredited college or university

  • Licensed as a Pharmacist in New York State or NYS internship permit

Preferred:

  • One year experience as a pharmacist

Work Environment

  • All work is performed in offices that require working in close quarters with other employees. The pharmacy settings are air conditioned / heated.
  • Adequate protective wear available for handling certain drugs (i.e., chemo) such as a mask, gown and gloves. Gloves and goggles are available for other procedures as necessary.
  • Distribution of Medications ‐ walking, standing, lifting, carrying, pushing, manual dexterity, reading
  • Drug Information / Clinical Activities ‐ sitting, talking, vision, fingering, manual dexterity, reading
  • Keyboarding / Computer ‐ Manual dexterity, sitting, grasping, vision, reaching 6"
  • Answering Telephones ‐ reaching 6", fingering, hearing
  • Compounding ‐ walking, standing, lifting, carrying, pushing, manual dexterity, reading
  • Equipment Used: carts, computer, printer, pharmacy robotics, phones and fax machines
  • Employees are required to have reasonable access to transportation to meet work requirements made in the ordinary course of business in a timely and efficient manner.
