Details Home health setting serving patients across multiple territories in the greater Richmond and Oakland, CA areas Primary territory covers Richmond, Rodeo, El Cerrito, El Sobrante, Hercules, Pinole, San Pablo, Crockett, and Kensington, with rotation into either the Oakland/Berkeley/Alameda area or the Vallejo/Napa area Occasional travel outside of the assigned service area may be required to maintain caseload expectations Day shift schedule; weekend coverage may be required Visit types include routine visits, evaluations, OASIS assessments, recertifications, discharges, and follow-ups Hours are calculated on a per-visit point system: most visit types = 1 point (1.33 hours); OASIS SoC = 1.5 points (2 hours) All-inclusive rate covers drive time, visit time, documentation time, and patient/physician phone calls Clinician will be provided a smartphone and medical devices including stethoscope, pulse oximeter, gait belt, and blood pressure cuff Requirements Active CA PT license required (license required if applicable) Minimum 1 year of physical therapy experience Current BLS certification required Valid driver's license, current auto insurance, and current auto registration required Annual TB test required MMR, Varicella, HepB, and TDap (within 10 years) immunization records required Annual flu vaccination required Drug screen and physical required Must be willing to see COVID-positive and COVID PUI patients Additional Information Responsibilities include conducting home health visits, completing OASIS assessments, evaluations, routine treatments, recertifications, and discharge planning Must be willing to travel outside the assigned service area or work weekends if needed to meet weekly visit expectations All requested time off (RTO) must be disclosed at the time of submission; RTO requests after the acceptance date will not be approved Former employees of the facility are not eligible for this travel assignment American Traveler Job ID P-695449.
