This vital role involves overseeing medical care, assisting with Activities of Daily Living (ADLs), and supporting residents in a 6-bed Intermediate Care Facility (ICF). Key Responsibilities:

• Supervise and coordinate medical care for residents with IDD
• Assist residents with ADLs and other daily activities
• Monitor health status and report changes to the healthcare team
• Maintain accurate documentation of care provided
• Collaborate with interdisciplinary teams to ensure individualized care plans

Required Skills:

• Valid California LVN license
• Experience working with individuals with intellectual and developmental disabilities
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Knowledge of medical protocols and medication administration

Nice to Have Skills:

• Prior experience in an ICF setting
• CPR and First Aid certification
• Bilingual abilities (English/Spanish)
• Experience with electronic health records (EHR) systems

Preferred Education and Experience:

• Minimum of one year of LVN experience, preferably in a long-term care or developmental disabilities setting
• Relevant certification in healthcare or disabilities support

Other Requirements:

• Valid California LVN license and CPR certification
• Ability to pass background checks and health screenings
• Flexibility to work various shifts, including weekends and holidays
• Willingness to travel or work at different facilities if required
