This vital role involves overseeing medical care, assisting with Activities of Daily Living (ADLs), and supporting residents in a 6-bed Intermediate Care Facility (ICF). Key Responsibilities:
- Supervise and coordinate medical care for residents with IDD
- Assist residents with ADLs and other daily activities
- Monitor health status and report changes to the healthcare team
- Maintain accurate documentation of care provided
- Collaborate with interdisciplinary teams to ensure individualized care plans
Required Skills:
- Valid California LVN license
- Experience working with individuals with intellectual and developmental disabilities
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Knowledge of medical protocols and medication administration
Nice to Have Skills:
- Prior experience in an ICF setting
- CPR and First Aid certification
- Bilingual abilities (English/Spanish)
- Experience with electronic health records (EHR) systems
Preferred Education and Experience:
- Minimum of one year of LVN experience, preferably in a long-term care or developmental disabilities setting
- Relevant certification in healthcare or disabilities support
Other Requirements:
- Valid California LVN license and CPR certification
- Ability to pass background checks and health screenings
- Flexibility to work various shifts, including weekends and holidays
- Willingness to travel or work at different facilities if required
