MultiCultural Home Care, a Modivcare Personal Care Service, is looking for a Care Coordinator Associate. In this role, you’ll play a meaningful part in helping patients and their families navigate care services—making a real difference every day by ensuring care needs are met smoothly and with care.
Pay: $24.00/Hourly
Schedule: M-F 8:30AM-5:00PM

What You’ll Do
Receiving and processing referrals from hospitals, physicians, and other healthcare partners
Working closely with internal teams to determine eligibility and begin the intake process
Serving as a friendly, knowledgeable point of contact for patients, families, and healthcare professionals
Answering questions, addressing concerns, and guiding patients to the right services and resources
Scheduling appointments while balancing provider availability and patient preferences
Managing schedule changes, cancellations, and reschedules with care and attention
Sharing important patient information with the appropriate care team members
Maintaining accurate and organized patient records
Providing compassionate, empathetic support throughout the care coordination journey
What We’re Looking For
High school diploma or equivalent
1+ years of customer service experience required.
1+ years of home healthcare experience preferred.
English & Spanish speaker required
