Monday-Friday, 8:00 AM – 4:30 PM (primary inpatient days); Emergency Department coverage as needed 10:00 AM – 8:00 PM; 8–10 hour day shifts, guaranteed 40 hours/week; weekends approximately every 3–4 weeks — Day shifts

Roles & Responsibilities
Conduct comprehensive social work assessments and mental health evaluations for diverse inpatient populations
Manage case loads of roughly 12–16 consults per day with patient‑centered care coordination
Develop and coordinate safe, practical discharge plans in collaboration with interdisciplinary teams
Initiate Title 36 petitions and complete required DCS/APS reporting when indicated
Address homelessness, substance use concerns, and social determinants of health with referrals and community resources
Perform maternal‑child assessments and provide support in the Family Birthing Center
Document thoroughly and accurately in Cerner to ensure continuity of care
Provide crisis intervention and timely communication with managers and clinical teams
Qualifications
Licensed LMSW or LCSW required
Minimum ~1 year of relevant social work experience (inpatient, ED, ICU or maternal‑child experience valued)
Experience with Cerner charting strongly preferred
Demonstrated ability to complete Title 36 petitions and mandatory reporting (DCS/APS)
Comfort working across inpatient, ED, ICU and Family Birthing Center settings
Willingness to undergo profile review and manager interview; must meet radius/employment eligibility per posting
Knowledge & Skills
Clinical social work and case management best practices

Strong documentation skills in Cerner
Knowledge of SDOH screening and community resource linkage
Experience with Title 36 procedures, DCS and APS reporting
