Summary
The Homemaker/Home Health Aide Services (H/HHA) are personal care and related support services that enable frail or disabled veterans to live at home. Home health care is part of the continuum of care provided to veterans. The Senior Social Worker may conduct home visits, video visits and telephone contacts after the placement of personal care services to provide continued oversight.
Qualifications
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Education: Having a master’s degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools in candidacy status do not meet this requirement until accreditation is completed. A doctoral degree in social work may not substitute for the master's degree.
Licensure/Certification: Senior social workers must be licensed or certified by a state at the advanced practice level and able to provide supervision for licensure.
English Language Proficiency: Candidates must be proficient in spoken and written English under 38 U.S.C. § 7403(f).
Grade Determinations GS-12 Experience: The candidate must have at least two years of experience post advanced practice clinical licensure, with one year at the GS-11 grade level.
Senior social workers must demonstrate possession of advanced practice skills and judgment.
Senior social workers are experts in their specialized area of practice.
Senior social workers may hold certification or post‑master’s training from a nationally recognized organization or university that includes a defined curriculum and supervised experience in a specialty.
Demonstrated Knowledge – Skills – and Abilities: In addition to the experience above, candidates must demonstrate skills in specialized interventions and modalities, individual/group/family counseling, advanced psychosocial and case management, complex differential diagnosis, effectiveness measurement, specialized consultation, policy writing, and advanced practice guideline development.
Preferred Experience: Experience working with geriatric populations and their caregivers, care coordination and case management for patients with chronic medical conditions and cognitive impairment in home and community settings, including oversight of services such as Homemaker Home Health Aide, Home Health Care, Hospice, or long‑term care; interdisciplinary collaboration and independent work.

Physical Requirements: No special physical qualifications are required. The work is basically sedentary.
Duties
Work closely with the home health agency to ensure timely and appropriate provision of services; review care plans and monitor services.
Provide solution‑focused case management and make appropriate referrals to resources when needed.
Make clinical determinations regarding services based on Veterans' needs and treatment goals.
Document all activities in Veterans' electronic medical records.
Coordinate in‑home respite care services to provide caregivers relief, as outlined in VA policy/handbook.
Provide clinical psychosocial assessments and solution‑focused case management at an advanced practice level to eligible veterans and family members receiving personal care services.
Conduct visitation, oversight, and monitoring per program guidelines.
Provide cross coverage for GEC Personal Care Services programs and other social workers during the home‑based work schedule (Monday – Friday, 8:00 am – 4:30 pm).

Parental Leave: After 12 months of employment – up to 12 weeks of paid parental leave for birth, adoption, or foster placement of a child.
Child Care Subsidy: After 60 days of employment – full‑time employees with a total family income below $144,000 may be eligible for a subsidy up to 25% of eligible childcare costs, up to a monthly maximum of $416.66.
Retirement: Traditional federal pension (5 years vesting) and federal 401(k) with up to 5% contribution by VA.
Insurance: Federal health, vision, dental, term life, and long‑term care.
