As a Referral Coordinator you’ll aid providers with continuity of care by ensuring referrals get submitted promptly to high‑value specialists and close the loop of care by obtaining consult notes after specialist visits. This is a full‑time role involving 8‑hour weekday shifts with no weekends. The position works a hybrid schedule with 3 days remote and 2 days onsite. We operate in a busy, fast‑paced environment, and we seek a candidate who thrives under such conditions.

Responsibilities

Ensure referrals are submitted, accepted, and followed up on in a timely manner.

Provide appropriate clinical information to outside entities, as needed.

Ensure complete and accurate registration for patients.

Assist patients in problem‑solving issues related to their referrals, including taking an advocacy role for the patient with the carrier to ensure approval of necessary services in a timely fashion.

Maintain detailed and accurate referral documentation and communication with referring providers within the electronic medical record.

Establish and maintain relationships with providers and outside entities.

Review details and expectations about the referral with patients as needed.

Ensure the patient’s chart is up to date with consult information from other entities.

Monitor and ensure proper utilization of the High Value Specialist and Medical Neighborhood networks so patients have access to the right care at the right time.

Communicate effectively with specialist offices to assure safe and coordinated transitions of patient care.

Qualifications
Candidates should have a High School Diploma, or an associate’s degree in healthcare administration or a related field (preferred). Along with medical terminology knowledge and 1‑year related experience (preferred).
