Overview

The Wellness and Resource Coordinator - Part-Time role, in our Family Hub program will create, coordinate and facilitate wellness and activities and programs as well as provide mental health screenings, trainings, resources and service referrals to Family Hub clients, parents and families of our early education centers and senior services.

Position Summary
The Wellness and Resource Coordinator - Part-Time role, in our Family Hub program will create, coordinate and facilitate wellness and activities and programs as well as provide mental health screenings, trainings, resources and service referrals to Family Hub clients, parents and families of our early education centers and senior services.
Education & Certification Requirements

Master’s degree in social work, counseling, or related field. 2-years, or more working in/for non-profit organization(s).
Health and Wellness certification a plus.
Experience & Training Requirements

Bilingual in Spanish required.
Exceptional verbal, non-verbal and written communication skills
Ability to manage multiple projects simultaneously.
Have considerable knowledge of health and wellness activities.
Be culturally competent with an understanding of the populations we serve.
Must be comfortable speaking to and/or instructing small groups.
Received training in counseling interventions focused on families and young children.
Utilize motivational strategies to support overall family health and wellness.
Proficient in Microsoft Office, data analysis, and curriculum development.
Desired Outcomes

Track and increase improved health and wellness outcomes related to improvement in health literacy, stress management and coping skills, family functioning, nutrition education, home remedies, and food consumption with 20-new clients and families each quarter.
Create a resource manual including health and wellness resources, referral contacts for counseling and crisis services used by Family Hub.
Track and provide one-on-one counseling and family counseling services to 20 clients every month.
Respond to all referrals made for health and wellness services to the Family Hub within 48-hours of receipt.
Create outreach plan to increase client and family participation which will be updated at the end of each fiscal year.
Essential Job Requirements & Duties

Create and document health and wellness services for Family Hub clients, and families, early childhood education and senior services families, and staff.
Supervise and coordinate health promotion activities and wellness workshops, programming, and coordination of services.
Instructs and conducts educational trainings in an effort to promote safe and healthy lifestyles.
Provide mental health assessment and screening for individual, family, or parent child counseling services.
Provide crisis intervention as needed to determine the necessary level of intervention and provide referral resources.
Conduct parent support groups in collaboration with master’s and bachelor’s degree interns.
Coordinate ongoing activities and programming with interns and external consultants.
Recruit high-quality instructors/consultants to provide complementary workshops/classes.
Actively promote and recruit for health and wellness programming
Research best practices and create 2-Gen’ programs that engage children and their caregiver(s)
Provide direct opportunities like wellness challenges and health coaching.
Contribute to effective communication tools/marketing materials
Work with the Family Hub, Senior Services, and Family Workers to develop wellness campaigns to promote a healthy lifestyle and generate family participation.
Assist with research, evaluations, and implementation of best practices in wellness trends and health education to engage parents and colleagues, to promote a culture of wellness
Assist with the coordination of wellness committee activities, providing tactical support and coordination for monthly onsite health promotion programs
Work with the development team to collect adequate data and contribute reports for funders
Must maintain clear objectives to meet deliverables.
Track impact by developing a system to assess the effectiveness of wellness programs including the creation of the evaluation tool(s)
Conduct surveys to determine interest in, or satisfaction with, wellness programs, events, and services
Develop partnerships with local community organizations/resources.
Represent Chicago Commons in Health and Wellness meetings and related coalitions.
Physical Requirements
