Practice Structure:

• patients per day
• Minimal phone call approximately 1 2 times per month
• Integrated behavioral health collaboration
• Multidisciplinary care teams
• Strong administrative and care coordination support

Clinical Scope:

• Full-spectrum outpatient primary care
• Chronic disease management
• Preventive health services
• Long-term patient relationship building
• Quality improvement participation

Compensation & Benefits:

• $220K $275K salary
