Physiatry/Physical Medicine & Rehabilitation Physician

Coverage Needed :

• Schedule: Full-time, Monday to Friday, 8:00 AM - 4:30 PM
• On Call: Every 3rd weekend, Saturday or Sunday 8:00 AM - 4:30 PM rounding in person, with call from Saturday 12:00 AM to Monday 12:00 AM
• Open Weeks:
  • May: 5/18-5/22
  • June: 6/1-6/7, 6/8-6/12, 6/15-6/21, 6/22-6/26, 6/29-7/3
  • July: 7/6-7/12, 7/13-7/17, 7/22-7/26, 7/27-8/2
  • August: 8/3-8/7, 8/10-8/14, 8/17-8/23, 8/24-8/28, 8/31-9/4
  • September: 9/7-9/11

Clinical Details :

• Inpatient only
• Patient Volume: 12 - 15 inpatients
• Procedures: Standard core privileges, General PM&R
• Rounding
• EMR: EPIC (experience preferred, training available)

Requirements :

• Active Indiana license and DEA
• Board Certified or Board Eligible
• Willingness to cross-credential for potential work in 2nd location
• CV must be in Month / Year format
• Must have license in hand at time of submission, or proof that the license application has been submitted
• Any gaps greater than 30 days will require an explanation on the CV
• DEA must remain in state to maintain privileges. Provider must be willing transfer and maintain DEA in state for the duration of privileges being active, or obtain a new in state DEA to remain in state for the duration of privileges being active
