Qualifications

Associate's degree, with a focus on science or social sciences.

One year of experience with public health services.

Preferred: Experience and/or education in social work, experience with public health and service systems.

Valid Illinois driver's license, auto insurance, and reliable transportation.

What You’ll Do

Connect individuals and families to health, social, and community services while providing culturally relevant outreach and education.

Support clients in navigating health and human service systems and help reduce barriers such as transportation, language, or technology.

Conduct community outreach to identify people eligible for SNAP and provide screenings, application assistance, and document verification.

Prepare clients for IDHS interviews and support recertification, case follow-up, and troubleshooting.

Assist clients with meeting work requirements, exemptions, or referrals to supportive programs.
