Your Role in Our Mission
Easterseals PORT Health’s Tailored Care Management (TCM) team is committed to delivering whole-person, integrated care that truly makes a difference. As a
Tailored Care Manager , you’ll play a vital role in supporting individuals by evaluating their needs, developing personalized care plans, and coordinating services that address physical and mental health, social determinants of health, and other life challenges. This is a meaningful, hands‑on role with the opportunity to
travel throughout Wilmington, NC and surrounding counties , helping close gaps in care and improving lives in the community every day.

$53,000 to $56,000
annually, based on your experience and qualifications. This
full-time
role offers a

What We’re Looking For
We’re seeking a compassionate, driven professional with a passion for helping others and connecting clients to vital community resources. This role requires flexibility to provide services in various community settings and participate in rotating after‑hours support.
We also require:
Bachelor’s degree
in a field related to health, psychology, sociology, social work, nursing or another relevant human services area. Master’s level degree preferred.
Eligible for
Qualified Mental Health Professional (QMHP)
credentialing
Minimum 2 years of experience
with mental health and/or substance use populations
Proficiency in Microsoft Office and Electronic Health Records (EHR)
Valid driver’s license, current auto insurance, and a clean driving record
Strong communication skills and the ability to work independently
