About the Role
As a
Tailored Care Manager , you’ll play a vital role in supporting individuals by evaluating their needs, developing personalized care plans, and coordinating services that address physical and mental health, social determinants of health, and other life challenges. This is a meaningful, hands‑on role with the opportunity to travel throughout Wallace and Kenansville, NC helping close gaps in care and improving lives in the community every day.
Compensation and Schedule
You’ll earn a competitive salary ranging from
$53,000 to $56,000
annually, based on your experience and qualifications. This
full‑time
role offers a
non‑traditional Monday‑Friday schedule , with rotating on‑call responsibilities. We value work‑life balance and strive to provide a supportive environment that allows you to manage your professional and personal commitments effectively.

Qualifications
Bachelor’s degree in a field related to health, psychology, sociology, social work, nursing or another relevant human services area. Master’s level degree preferred.
Eligible for Qualified Mental Health Professional (QMHP) credentialing
Minimum 2 years of experience with mental health and/or substance use populations
Proficiency in Microsoft Office and Electronic Health Records (EHR)
Valid driver’s license, current auto insurance, and a clean driving record
Strong communication skills and the ability to work independently
