Easterseals PORT Health is seeking a full-time Registered Nurse (RN) to join our Assertive Community Treatment (ACT) Team. In this role, you’ll provide essential, community‑based care to adults living with serious mental health and substance use challenges.
This is not traditional facility‑based nursing. It’s impactful, hands‑on behavioral health work serving individuals where support matters most – in homes, shelters, and community settings.

Your Role in Our Mission
As an ACT Team RN, you will work within a multidisciplinary team delivering community‑based behavioral health support. You’ll provide psychotropic medication management, holistic nursing assessments, care coordination, and crisis response – all aimed at helping individuals achieve stability, independence, and improved quality of life.
You will develop meaningful relationships with clients, support them through real‑life challenges, and ensure seamless continuity of care across community environments. You’ll collaborate closely with psychiatrists, therapists, co‑occurring disorder specialists, and other behavioral health professionals to deliver truly integrated care.

This full‑time role follows a Monday–Friday schedule with rotating on‑call responsibilities. Community travel is required.
Compensation & Benefits
Salary Range:
$75,000 – $90,000
annually, based on experience.

What You’ll Do
Medication & Clinical Care
Administer and monitor psychotropic medications as prescribed
Conduct nursing assessments and track symptoms, side effects, and medical issues
Document all care and observations in the Welligent EMR
Care Coordination
Partner with the team psychiatrist to update medication protocols and care plans
Participate in daily in‑person ACT Team meetings to ensure coordinated, person‑centered treatment
Crisis Response
Provide crisis assessment and intervention as part of a rotating on‑call schedule
Community‑Based Support
Deliver nursing care in clients’ homes and community settings
Educate individuals on health management, wellness, and medication adherence
Advocate for client choice and connect individuals with needed community resources
What We’re Looking For
To be successful on the ACT Team, you should be compassionate, flexible, and comfortable providing care in diverse community environments.
Required Qualifications
Active, unrestricted RN license in North Carolina
Experience in behavioral health or mental health (preferred)
Comfort working in community‑based settings
Valid driver’s license, reliable transportation, and vehicle insurance
Strong communication, documentation, and relationship‑building abilities
