Travel Physical Therapist(PT) - Long Term Care (LTC) (Days)

Mobile Health Team Inc. is seeking an experienced Physical Therapist(PT) - Long Term Care (LTC) for a 13-week travel assignment in Lewistown, MO.

M-F, 8 hr days- Times are flexible as long as consistent and communicated with DOR. We do ask the therapists start time is before 6AM and no later than 10AM/ 35 GWW

Start Date: 5/18/2026

Contract: 13 weeks

Location: Lewistown, MO

Hourly Pay: $28.50

Non-taxed Stipend: $1246.00

Weekly Total Pay: $1994.13

Nonbillable hours: 8 hours

Weekend Requirements: No weekends unless a holiday falls within M-F, then they can make the hours up on the weekend.

This position follows AHSA's clinical interview process. Candidates can expect outreach within two hours—via phone and text where applicable—with decisions typically made the same day or by the next business day. Applicants who are looking for more than 7 days off, who are seeking more than 50% of holidays off, and/or have negative background or employment history are not eligible for the same processing timeline. If a new grad, candidate will not get the clinical interview process, they will be presented to the facility for review.

Key Responsibilities

Effectively screens/evaluates/rehabilitates patients with physical functioning disorders and/or wound care and develops appropriate plan of care following all regulatory and clinical practice standards.

Utilizes standardized assessments and evidence-based practice to support clinical interventions.

Administers various procedures as part of the rehabilitation plan including but not limited to: manual techniques; ambulation; therapeutic exercises; modalities; use of supportive and assistive devices; wound care; etc.

Assesses patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.

Evaluates treatment outcomes for effectiveness. Services are modified in relation to patient evaluation and referrals are made in relation to outcomes.

Provides consultation and counseling to patients, as well as families, caregivers and other service providers related to the physical disorders.

Effectively communicates with supervisor and other health team members regarding patient progress, barriers, and treatment plans.

Provides comprehensive discharge summary of services provided per regulatory and clinical requirements.

Documents all regulatory and clinically required information in the electronic documentation system and maintains patient records according to company policy.

Accurately records treatment minutes for services provided per all regulatory and clinical requirements.

Participation in required meetings including but not limited to: patient care conferences, utilization review meetings, rehabilitation meetings, etc.

Participate in facility and company required training, in-services and conferences.

Able to obtain any necessary DME or supplies per regulatory and clinical practice requirements.

Able to supervise and support physical therapist assistants; rehabilitation aides and/or students.

Adherence to APTA and State Required practice acts, code of conduct, ethics during professional practice as a Physical Therapist.

Compensation

Weekly Pay (hourly) + Non-taxed Stipend (if eligible)

Requirements

1 year of PT experience - Required

LTC or Rehab experience - Required

MO State License - Required

New Grads OKAY- as long as a Level II experience and not just a Level I observation experience

Missouri State License is a requirement; Compact Allowed

Excellent problem-solving and critical thinking skills
