Physical Therapist – 41103

Evaluates all assigned patients and develops, implements and modifies treatment plans in accordance with patient needs, goals and physician direction. Responsible for delegation and direction of all discipline‑specific therapy services for those assigned patients. Ensures design of functional treatment programs enabling effective return to community activities. Serves as an educator of patients, families, students, coworkers and self. Performs other professional duties associated with the care of the patient, development of discipline‑specific therapy services and design of program delivery.

Responsibilities

Evaluate all assigned patients and develop individualized treatment plans.

Delegate and direct discipline‑specific therapy services.

Design functional treatment programs enabling return to community activities.

Educate patients, families, students, coworkers and self.

Maintain compliance with confidentiality and organizational code of conduct.

Adhere to safety protocols and promote a safe environment.

Complete comprehensive patient evaluations and develop goal‑focused plans to progress patients toward discharge.

Document medical records accurately and timely, including goal writing, functional indicators, letters of necessity and charge entry.

Participate in student program, provide supervision, develop learning objectives and complete student evaluations.

Create individualized professional development plans, pursue continuing education and participate in professional organizations.

Maintain professional licensure in good standing and meet continuing education requirements.

Apply discipline‑specific state practice act and regulatory compliance.

Follow organizational performance improvement methodology and contribute to improvement initiatives.

Qualifications

License in a relevant discipline with good standing.

Experience in evaluating patients and creating treatment plans.

Strong documentation skills and adherence to state, federal and insurance guidelines.

Ability to supervise students and develop educational objectives.

Commitment to ongoing professional development and ethical practice.

Knowledge of safety protocols and ability to promote a safe work environment.

Exceptional communication and teamwork skills.

Compensation
Pay Range: $46 – $51/hr
