Your Role as a Case Manager:
Coordinate resident care from admission through discharge planning
Complete discharge planning assessments within 48 hours of admission
Develop and manage individualized discharge plans of care
Serve as a liaison between interdisciplinary teams to ensure coordinated care
Educate residents and families on diagnoses, medications, and use of medical equipment using the Teach Back Method
Assist with Advance Directives and ensure appropriate documentation is obtained
Coordinate insurance updates and manage appeals for denials
Schedule follow-up appointments and arrange post-discharge services, including DME, medications, and home health
Ensure all discharge documentation and instructions are complete and communicated clearly
Conduct post-discharge follow-ups at 1 week and 30 days
Communicate discharge plans and updates with leadership and care teams
Supervise unit staff and support adherence to care standards and resident rights
Assist with activities of daily living as needed
Maintain a safe, clean, and supportive environment for residents and staff
What We're Looking For:
Graduate of an accredited School of Nursing
Current LPN or RN license in the State of Ohio
Current CPR certification
5+ years of experience in healthcare or long-term care preferred
Strong care coordination, organizational, and problem-solving skills
Excellent communication and interpersonal abilities
Ability to work independently and collaboratively with a team
Compassionate, patient-focused approach to care
Ability to manage multiple priorities in a fast-paced environment
