Your Role as a Social Services Coordinator
Provide social services support to residents, addressing psychosocial, emotional, and environmental needs.
Communicate with residents and families to resolve concerns, provide guidance, and ensure needs are met.
Complete and maintain comprehensive assessments, including social histories, care plans, and progress notes.
Advocate for residents’ rights, ensuring dignity, privacy, and respect at all times.
Collaborate with interdisciplinary teams, including staff, consultants, and healthcare professionals.
Maintain strict confidentiality of resident information in accordance with HIPAA regulations.
Support a calm, caring, and respectful environment for residents and staff.
Participate in care planning and contribute to individualized resident care strategies.
Attend required trainings, in‑services, and meetings.
Qualifications
Bachelor’s degree in Social Work or a related field from an accredited college or university.
Current Social Worker license in the State of Ohio.
Minimum of 2 years of experience in a healthcare setting preferred.
Strong problem‑solving, organizational, and multitasking skills.
Excellent communication and customer service abilities.
Ability to maintain accurate and timely documentation.
Ability to work both independently and collaboratively as part of a team.
Compassionate, calm, and professional demeanor in emotionally demanding situations.
Ability to read, write, and speak English.
