How You’ll Make an Impact in this Role
Direct Patient Care (ADLs):

Assist patients with personal hygiene (bathing, grooming, linen changes), nutritional support (feeding, water, I/O documentation), and elimination needs (toileting, dignity maintenance).

Mobility and Safety:

Safely assist with patient transfers, positioning, and ambulation using proper mechanics and assistive devices. Actively implement fall prevention strategies (bed alarms, side rails, call lights).

Observation and Communication:

Accurately monitor and record vital signs. Continuously observe and immediately report significant changes in a patient's physical or mental status to the RN/LPN (e.g., pain, confusion, breathing difficulty, skin changes). Document all care and observations accurately in the EHR.

Environmental Care:

Maintain a clean, safe, and orderly patient environment. Care for and ensure proper function of medical equipment. Strictly adhere to all infection control and PPE policies.

Minimum Requirements You’ll Need
Licensure / Certification / Registration:

BLS Provider obtained within 1 month of hire date or job transfer date required. American Heart Association or American Red Cross accepted.

Nurse Aide credentialed from the Wisconsin Nurse Aide Registry obtained prior to hire date or job transfer date required. Certified Nursing Assistant credentialed from the state of Wisconsin, and in Nurse Aide Registry with eligibility to work in a hospital setting obtained prior to hire date or job transfer date. Additional certs may be required by specific job locations.

Education:

High School diploma equivalency OR 1 year of applicable cumulative job specific experience required.

Note: Required professional licensure/certification can be used in lieu of education or experience, if applicable.
