How you’ll make an impact in this role
Lead patient care journeys by implementing and monitoring personalized care plans that drive measurable recovery outcomes.
Coordinate the multidisciplinary team to ensure every specialist and support service is aligned for seamless, high-quality patient transitions.
Monitor and communicate patient status in real-time, providing the care team with the critical insights needed for timely clinical decisions.
Advocate for safe discharges by assessing and preparing patients for their next stage of healing, ensuring they and their families feel confident and supported.
What Minimum Requirements You’ll Need
Licensure / Certification / Registration:
BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
Licensed Registered Nurse credentialed from the Texas Board of Nursing required.
Education
Diploma from an accredited school/college of nursing required OR a bachelors level degree not in nursing, an active RN License, and enrolled in an Alternate Entry Masters of Science in Nursing Program with a minimum of 1 year in the program completed.
What Additional Preferences We're Seeking
Experienced Registered Nurse with 2 years of clinical expertise, demonstrating a proven ability to thrive and maintain high standards of care within high-volume, fast-paced medical environments.
Highly organized professional with exceptional multitasking and communication skills, dedicated to streamlining clinical workflows and ensuring seamless coordination within multidisciplinary teams.
