Coverage Needed

• Schedule: 2 weeks on/2 weeks off with call
  • Clinic Hours: 8 AM - 5 PM
  • Call TBD

  • September 14 - October 2 (3 weeks)
  • October 19 - 30 (2 weeks)
  • November 16 - 27 (Excludes Thanksgiving) (2 weeks)

• 95% Outpatient, 5% Inpatient
• Number of Physicians: 1
• Ancillary Staff per Provider: 1 RN or CMA per physician
• Number of Exam Rooms per provider: 2
• Medical Imaging, Lab & Pharmacy
• Clinic Provider Daily Volume: 10-30
• Average Daily Hospital Practice Census: 10-15 Primary, 5-8 Secondary
• Appointment Length: 20 Minutes
• Patient Population: Primary Clinic; Medicare/Medicaid/Commercial; Secondary Clinical; Commercial, Medicare
• EMR: EPIC
• Teaching Opportunity: Informal
• Services offered in the practice: Patient assessments, new treatment plans, ongoing treatment plans, changes in therapy, referral for ancillary services, end of life care, chemotherapy management, pain management (radiation and surgical oncology management provided by existing providers with strong relationships)

Requirements

• Board Certified or Truly Board Eligible in Hematology and Oncology
• Willing to review candidates Board Certified in Oncology with Hem/Onc experience & willing to see Hematology patients
• Active IA License or Active IMLC
• Fellowship Trained in Hematology & Oncology
• Must be experienced
• Provider must be willing to work at both locations
• Clean Malpractice/Background
• Self-Query NPDB Report pulled within 30 days
