This role emphasizes patient-centered care, teamwork, and quality improvement, with opportunities for leadership and teaching. Position Highlights Outpatient primary care-focused role Collaborative, team-based environment Opportunity to mentor healthcare students Strong emphasis on Patient-Centered Medical Home (PCMH) model Mission-driven care focused on underserved populations Clinical Responsibilities Provide comprehensive primary care services Diagnose and treat acute and chronic conditions Utilize evidence-based clinical guidelines Collaborate with physicians, behavioral health, and care teams Maintain accurate and timely EMR documentation Participate in quality improvement and patient safety initiatives Leadership & Program Involvement Support implementation of clinical workflows and quality measures Participate in PCMH and federal reporting initiatives Conduct chart reviews and evaluate quality of care Assist with staff education and clinical training Present clinical topics in provider meetings Collaborate with leadership on clinical goals and operations Additional Responsibilities Participate in staff meetings and organizational initiatives Ensure compliance with HIPAA, OSHA, and accreditation standards Travel between clinic sites as needed Maintain required certifications and licensure Qualifications Licensed Nurse Practitioner or Physician Assistant Active or eligible Wyoming license DEA certification BLS/CPR (or ACLS) required Strong clinical judgment and communication skills Preferred Qualifications Experience in primary care or community health Familiarity with EMR systems (Athena preferred) Bilingual (English/Spanish) Practice Environment Community health center setting Team-oriented, mission-driven culture Focus on access, quality care, and population health Physical & Work Environment Routine clinical activity (standing, walking, patient care tasks) Exposure to standard healthcare environments including infectious disease risk
