Job Summary
The Speech Language Pathologist (SLP) provides skilled speech therapy services in a Home Health setting. This role focuses on evaluating, diagnosing, and treating communication and swallowing disorders while collaborating with interdisciplinary teams to develop and implement individualized care plans.
Key Responsibilities
Provide direct speech therapy services to patients in a home health setting
Evaluate and treat speech, language, cognitive, and swallowing disorders
Develop and implement individualized treatment plans
Educate patients and caregivers on therapy techniques and home programs
Collaborate with physicians and interdisciplinary teams
Maintain accurate and timely documentation
Utilize rehabilitation equipment such as communication aids and assistive devices
Work independently while managing patient caseload efficiently
Requirements
Minimum 2 years of Home Health SLP experience – Required
Ability to work independently with minimal supervision
Valid Driver’s License – Required
Must have own vehicle (mandatory for this role)
Acute care experience – Preferred
Education
Master’s or Doctorate Degree in Speech Language Pathology – Required
Certifications & Licensure
CCC (Certificate of Clinical Competence) – Required (Clinical Fellows accepted)
BLS (AHA) – Required
Active Massachusetts SLP License – Required
