Description
The Clinical Pharmacist Specialist will collaborate with physicians and staff to provide coordinated, comprehensive, high quality care to patients.
Required Qualifications
Bachelor's Degree in PHARMACY from an accredited School of Pharmacy or
PharmD in PHARMACY from an accredited School of Pharmacy.
Coursework/Training – Postgraduate Year One (PGY-1) AND Year Two (PGY-2) Pharmacy Residency or equivalent experience or education.
Oregon Pharmacist License upon hire.
Certification from Board of Pharmacy Specialties within 2 years of hire.
TWO years of Senior Ambulatory Clinical Pharmacist experience within relevant practice setting – OR – FIVE years of pharmacy practice experience with at least 2 years of experience in a relevant setting in lieu of PGY-1 and PGY-2 residency.
Preferred Qualifications
Board of Pharmacy Preceptor license when eligible and where applicable upon hire.
