Job Description
Seeking an experienced Social Worker Case Manager with a minimum of 2 years acute care experience in a hospital setting to manage a caseload of 15‑18 patients, with potential adjustments based on complexity. Requires strong generalist skills and flexibility across various hospital units.
Responsibilities
Manage a caseload of patients across multiple hospital areas including Cardiac/Pulmonary, ED, ICU, MedSurg, Oncology, Ortho, and Women & Children’s units.
Conduct psychosocial assessments, discharge planning, competency and mental health evaluations, suicide risk assessments, and safety planning.
Collaborate interprofessionally and adapt to different units and assignments.
Utilize documentation and communication skills effectively.
Engage with social drivers of health, including cultural diversity, bilingual care, homelessness, substance use disorder, and poverty.
Requirements
Experience in Mother Baby or ED for the current contract.
LICSW WA license highly preferred, will accept LASW, LSWAIC, LSWAA, or LICSW-P (probationary).
Ability to float throughout the hospital and work weekend rotations.
Familiarity with EPIC EMR system.
Soft skills in time management, multitasking, and collaborative practice.
