The State of Connecticut, Department of Developmental Services (DDS) has an opportunity for a Clinical Nurse Coordinator based in Wallingford, CT. This position will be assigned to the Department of Developmental Services Public Programs South Region and is essential to meeting the health and safety needs of individuals living at the DDS Public Community Living Arrangements.
Position Highlights
Location: Wallingford, CT
Shift: Full-time | first shift | 40 hours per week
Schedule: 8:00 AM – 4:30 PM, Monday through Friday
Once the employee completes a 6‑month working test period, this position will be available with a hybrid work schedule. Scheduling flexibility is required to meet agency needs.

Your Role
As a Clinical Nurse Coordinator, you will dedicate your time and care to the population within the agency’s Community Living Arrangements (CLAs). CLAs, also known as group homes, are operated by the DDS or private agencies that allow individuals to live in community housing. Homes are small in size and generally serve six or fewer individuals.
In this role, you will:
Provide oversight and coordination of the medical needs of the residents;
Provide nursing assessment and interventions;
Delegate nursing procedures and medication administration to non‑licensed staff;
Conduct assessment of the effect of psychotropic medications for efficacy;
Provide ongoing and incidental training to the direct care staff working with the residents.
The majority of CLA residences are enrolled under a Medicaid Home and Community Based Waiver administered by DSS to capture federal reimbursement.

Minimum Qualifications – General Experience
Three (3) years of professional nursing experience.
Minimum Qualifications – Substitutions Allowed
A Bachelor’s degree in nursing may be substituted for one (1) year of the General Experience.
A Master’s degree in nursing may be substituted for one (1) additional year of the General Experience.
Preferred Qualifications
Nursing experience working with individuals with Intellectual and/or Developmental Disabilities, autism spectrum disorders, with concurrent behavioral and mental health diagnoses.
Experience with basic nursing skills, including medication and nutrition via G‑tube or J‑tube, diabetic care, respiratory treatments, colostomy, cecostomy, urostomy care, wound care, catheterization emergent care, and hospice care in the home.
Experience following current infection control practices.
Experience with clinical documentation, including documenting events in writing.
Experience working in community and family home settings and coordinating in‑home supports.
Experience reviewing health information and formulating reports based on that information.
Experience using Microsoft applications, including Outlook, Teams, Word, and Excel.
Experience communicating, interacting with, and explaining information to internal partners, external partners, and the public.
Experience working in a licensed facility.
Special Requirements
Incumbents in this class must possess and retain a license as a Registered Nurse in Connecticut OR hold a Multistate or Compact Registered Nurse license.
Incumbents in this class may be required to travel.
