Responsibilities
Provide direct nursing care to residents under supervision of a registered nurse.
Administer medications and treatments per physician orders; document responses and any adverse effects.
Monitor vital signs, assess changes in condition, and elevate appropriately per policy.
Assist with activities of daily living and maintain resident safety, comfort, and dignity.
Collaborate with the interdisciplinary care team to implement and update care plans; communicate changes to families as appropriate.
Maintain accurate, timely documentation and uphold infection prevention and control protocols.
Requirements
Active Licensed Practical Nurse license in New Hampshire (or eligibility to practice in NH).
Current CPR/BLS certification.
Ability to work evenings; effective communication and documentation skills.
Experience in a long‑term care or skilled nursing setting preferred.
Graduation from an approved practical nursing program.
