About the Role
We are seeking a compassionate and experienced Nurse Practitioner to provide high-quality clinical care across weekend shifts in Richmond, VA. This role is ideal for a clinician who values autonomy, collaboration, and patient-centred care within post-acute and long-term care environments.
Schedule
This position covers weekend shifts (Saturday and Sunday). Flexible shift patterns may be available based on coverage needs.
Core Responsibilities
Deliver comprehensive medical care to residents in post-acute, long-term care, and assisted living settings during weekend coverage
Perform thorough assessments, develop and implement treatment plans, and manage acute and chronic conditions
Prescribe medications and manage medication adjustments within scope of practice and state regulations
Order and interpret diagnostic tests, and coordinate follow-up care with specialists and primary teams as required
Document patient encounters accurately and promptly in the Electronic Medical Record (EMR)
Collaborate with nursing staff, physicians, therapists, and other allied health professionals to ensure continuity of care
Participate in care planning, case reviews, and quality improvement initiatives
Provide clear communication to families and facility staff regarding clinical status and care plans
Qualifications and Required Skills
Master’s or Doctoral degree as a Nurse Practitioner from an accredited institution
Active, unrestricted Virginia NP licence and current DEA registration
Current certifications as required by role (e.g., BLS) and facility policies
Experience in post-acute care, long-term care, geriatrics, or related settings preferred
Proficiency with Electronic Medical Record systems
Strong clinical judgement, excellent communication skills, and the ability to work independently during weekend shifts
Compassionate, patient-centred approach and ability to build professional relationships with multidisciplinary teams

Consistently providing timely, accurate clinical assessments and appropriate interventions during weekend coverage
Maintaining clear, comprehensive documentation in the EMR
Fostering collaborative relationships with facility staff and families to support patient-centred outcomes
Contributing to quality improvement and ensuring safe transitions of care
