
DAY SHIFT LICENSED PRACTICAL NURSE

Licensed Practical Nurse (LPN) for Pediatric Group Home
Licensed Practical Nurse (LPN) is responsible for performing essential care for our members in our nursing supported group homes under the direction of the Chief Nursing Officer and NSGH Administrator. This is a safety sensitive position for a PEDIATRIC GROUP HOME. THIS IS FOR DAY SHIFT ONLY.

Responsible for Members which includes:
Provide for the safety, emotional and physical needs of all members
Ensure members are clean, well-groomed and dressed age appropriately and correctly for the season and environmental conditions
Transportation to and from day program as needed
Position and re-position every 1 hour while awake and every 2 hours while asleep and as needed
Help members cook meals and to eat as needed
Ensure daily that members are being emotionally and physically stimulated to the best of their ability
Assist with showering members to ensure a complete shower is given as needed
Responsibility for Medical Aspects of Care which includes:
Responsibility to the Nursing Supported Group Home Manager and Chief Nursing Officer for reporting all medical aspects of care and changes in conditions regarding the members in the home
Performing routine nursing duties, including:
Taking a recording temperature, pulse, and blood pressure
Comfort, grooming, and hygiene needs
Diet management
Collecting specimens
Patient activities
Passing medications
Reordering medications as needed (or ensuring Nursing Services Director is aware and needs to reorder)
Assisting in direct care of members to include, but not be limited to:
Showering members
Changing/Repositioning members
Cooking meals
Assisting in cleaning the home
Assisting in doing laundry
Safeguard equipment and supplies, and checks equipment apparatus or appliances furnished to resident in the course of treatment
Maintaining and ensuring that all documentation is completed before end of shift which includes but not limited to: incident reports, daily nursing notes, Med Sheets, etc.
Responsibility to the Company which includes:
Being flexible in hours worked and location worked in order to assist with staff coverage if needed
Ensuring that you maintain compliance in regard to:
Article 9
Prevention and Support
Fingerprint Clearance Card
CPR/1st Aid
Adhere to and follow all directives and policy and procedures handed down by the company
Ensure that all required documentation is completed before the end of shift.

Must have and maintain current Arizona State Practical Nurse license (In good standing)
Maintain standards of care as outlined in the Nurse Practice Act by Arizona State Board of Nursing
Flexible hours to work times as well as work locations to assist with staff coverage if needed
Reliable transportation to work at any location
Must have or be capable of passing and maintaining Article 9 and Prevention and Support within 90 days of hire
Must have or be capable of passing and maintaining CPR within 30 days of hire
Must have or be capable of obtaining and maintaining a Class 1 Fingerprint Clearance Card

Must be at least 18 years of age.
Must be able to maintain a class 1 fingerprint clearance card.
Must abide by company policies and procedures
Employees must be able to verbally communicate in English, i.e. be able to understand what is being said to him/her in English. Must be able to use the English language so that what he/she is saying to another person can be understood, in all emergency situations so that the safety of patients/clients, other employees and other individuals who are in our facilities conducting is not jeopardized. All employees must be able to read and comprehend English.
