RN Case Manager

Education/Experience Minimum Required

* Graduate of an approved RN program OR a non-approved program supplemented by content equal to an approved program.
* Two years' experience as a Registered Professional Nurse required OR equivalent of education and experience.

Preferred

* Bachelor's degree in Nursing.
* Med/Surgical/Certified home care experience. License/Certification

Minimum Required

* License and current registration to practice as a Registered Professional Nurse in New York State.
* CPR Certification required upon hire. Work Environment
* Work performed in Patient Homes in a wide variety of unpredictable, non-sterile environments (furniture, other residents, icy walkways, pets etc.).
* PPE: Gowns, masks, bags, gloves available as needed and per policy.
* Requires ability to: Lift and/or carry a nursing bag and plastic bin each weighing up to 15 lbs. in addition to a laptop weighing 4 lbs.; Walk up flights of stairs carrying nursing bag/supplies and laptop; Stand for 30 minutes or longer at one time while working; Lift, push, pull a minimum of 50 lbs. without restriction; Independently transfer or assist patients to move, stand or walk in a safe manner.
