Per Diem Physical Therapist
to join our team, delivering compassionate care at Assisted Living Facilities located in
Concord, NH.
This is your chance to empower the geriatric and senior population to regain strength, mobility, and independence while working in a supportive, team-oriented environment.
As a Physical Therapist, you will:
Evaluate patients
to assess strength, balance, mobility, and functional impairments
Develop and implement personalized treatment plans
that focus on improving mobility, reducing pain, and enhancing overall function
Deliver care at one or more assisted living locations , providing meaningful services to a diverse group of seniors
Provide care with a high level of independence
in assisted living communities, while always supported by the broader Engage team for guidance and resources
Educate patients and families
on home exercise programs and strategies for maintaining functional independence and a better quality of life
Document patient care services
in compliance with state and facility standards using EMR
Enhance professional growth and development
through participation in educational programs, in-service meetings, and workshops

About You
Graduation from an accredited physical therapy program
Active or pending state license to practice PT
An interest in working with the senior population
Superior customer service and interpersonal communication skills
Strong communication and interpersonal skills to work effectively with patients, families, and colleagues
