Facility:
Morris SNF
Salary Range:
$40.00–$63.00 per hour
Practice Setting:
Short Term Rehab, Assisted Living, LTACH, Long Term Care
New graduates are welcomed.
Position is full‑time (22.5+ hours/week) or part‑time opportunities available.
Responsibilities
Provide and direct resident rehabilitation care within all accepted practice methods, while meeting center, state, federal, and accrediting standards.
Evaluate resident needs, develop effective treatment plans for assigned residents.
Manage and coordinate treatment plans with staff and other disciplines.
Participate in screening of residents according to directed schedules and practices.
Provide feedback to the designated supervisor on clinical and rehabilitation issues.
Engage in center‑directed processes and services.
Maintain accurate billing records and clinical documentation to meet regulatory requirements.
Participate in in‑service training programs as requested.
May travel to other facilities to provide services on a limited basis.
Requirements
Master's or Doctorate Degree in Physical Therapy.
Current and active Physical Therapist license in the state of operation.
Experience or knowledge in older adult rehabilitation preferred.
Knowledge of governmental and regulatory guidelines in designated healthcare settings preferred.
