How You'll Contribute
This position is responsible for supporting the needs of the department with a focus on clinical, operational, and administrative excellence.
Essential Functions
Conduct individual and group therapy sessions to educate patients about psychological, emotional, or substance abuse problems.
Participate actively in the treatment planning process.
Provide family counseling sessions to ensure appropriate communication and involvement of family members and support groups.
Communicate actively with clients, families, and outside referral sources.
Communicate proactively with those involved in the patient’s treatment.
Collaborate with the interdisciplinary team, including physicians, utilization review, and nursing staff members.
Communicate effectively with the team members.
Communicate to UR on treatment and LOS issues.
Complete appropriate paperwork for clients in a timely, accurate, and complete manner.
Demonstrate appropriate crisis intervention and de‑escalation skills.
Act as a team member during crisis situations, assisting others to ensure patient safety.
Complete accurate assessments of patients utilizing clinical skills.
Assist the care coordination department as requested.
Complete all documentation thoroughly, timely, and in accordance with hospital standards.
Demonstrate thorough knowledge of confidentiality.
Complete releases appropriately.
Demonstrate knowledge of HIPAA regulations and abide by those standards.
Demonstrate working knowledge of all clinical regulations, including TJC, Medicaid, and state and federal laws.
Participate actively in the discharge, transition, and after‑care planning treatment processes.
Other duties as assigned.
Additional Information
Prior experience with psychiatric and chemical dependency patients.
Qualifications and Requirements
Education: Master’s degree in Social Work, Counseling, or equivalent required.
License: Current clinical or social work license as required by state regulations.
Certifications: CPR and De‑escalation certification required or obtained within 30 days of hire.
Location: Woodland Springs, Conroe, TX.
