MUST HAVE A CURRENT DRIVER'S LICENSE
Canadensis, PA – Monday through Friday 4:15 pm to 6:00 pm
Effort, PA – Fridays 2:00 pm to 6:00 pm and Saturdays 8:00 am to 6:00 pm
Duties and Responsibilities
Performs personal care and household duties (e.g., dressing, toileting, feeding, light housekeeping, hygiene, application and removal of splints, etc.) in home or school settings
Maintains a clean and safe environment
Provides emotional and behavioral support
Follows the plan of care
Interacts as a member of a multidisciplinary team
Reports any changes in the patient’s condition to agency
urse case manager
Ensures safety and supervision of patients
