Speech Language Pathologist (SLP)
Full-Time | Part-Time | PRN
We are seeking a licensed Speech Language Pathologist to provide skilled therapy services to residents in accordance with physician orders and care plans. This role focuses on improving resident function, mobility, communication, and quality of life.
Responsibilities
Evaluate residents and develop individualized treatment plans
Provide therapy services in accordance with physician orders
Document evaluations, treatments, progress notes, and discharge summaries
Collaborate with nursing and interdisciplinary teams
Participate in care planning and utilization review meetings
Educate residents, families, and staff on therapy goals and plans
Maintain productivity and documentation standards
Support regulatory compliance and survey readiness
Qualifications
Graduate of an accredited therapy program (SLP)
Current state licensure in SLP
Current Certification of Clinical Competence (CCC)
Long-term care experience preferred
Strong documentation and communication skills
Patient-focused and team-oriented
Physical Requirements
Standing, walking, bending, and assisting residents
Ability to lift and support residents during therapy
Exposure to clinical environments
