Overview
Aventura at West Park Hills is seeking a compassionate and skilled
PRN Speech-Language Pathologist (SLP/CCC)
to join our rehabilitation team in a skilled nursing and long-term care setting. This role is ideal for an SLP looking for flexibility while delivering high-quality, resident-centered care.
Responsibilities
Evaluate and treat residents with speech, language, cognitive, communication, and swallowing disorders
Develop, implement, and update individualized treatment plans in collaboration with the interdisciplinary team
Complete accurate and timely documentation using Net Health and PointClickCare (PCC)
Provide education to residents, families, and staff regarding treatment plans and safe swallowing strategies
Ensure compliance with facility policies, state regulations, and professional standards
Qualifications
Master’s degree in Speech-Language Pathology
Current Ohio SLP license
CCC-SLP required
SNF/LTC experience preferred (new grads with strong clinicals encouraged to apply)
Experience with Net Health and PCC preferred
Strong communication skills and a team-oriented, resident-first approach
