Job Summary
MUSC Community Physicians (MCP) is an entity within MUSC that provides healthcare to patients throughout the state of South Carolina. Under clinical practice guidance of the Social Work Supervisor, the Social Worker assists with coordination of the discharge process to ensure a successful transition in collaboration with the health care team to achieve targeted patient outcomes.
Job Responsibilities
The Social Worker (MSW) reports to the Social Work Manager of the Institute of Psychiatry. The role completes a psychosocial assessment on the patient and caregivers throughout the hospitalization and arranges outpatient follow-up as needed. Provides direct clinical social worker services in an acute care psychiatric setting for a complex caseload of patients including psychosocial assessment, active treatment (group therapy), discharge planning, and case management. Identifies risk factors that the patient may experience after discharge and applies discharge interventions to lower the patient risk of an undesirable event post discharge. Bridges the discharge to outpatient provider with the patient and caregiver to address issues and questions.
Qualifications
Master's degree in social work or social welfare program accredited by the Council on Social Work Education.
1 year post graduate professional social work experience; 3 years preferred.
LMSW/LISW licensure by the South Carolina Board of Social Work Examiners. LISW-CP licensure preferred.
Excellent communication skills, attention to detail, and ability to work independently.
Ability to collaborate in a positive, professional manner with medical professionals.
Certified in Safety Care and Basic Life Support within the first 90 days of employment and annually thereafter.
