
PT - Physical Therapist

The Outpatient Physical Therapist is responsible for evaluating and developing comprehensive treatment plans tailored to each patient's needs and goals. This role involves the delegation and direction of therapy services while ensuring a focus on effective community reintegration. The therapist will educate patients, families, and peers, uphold professional standards, and maintain compliance with organizational policies.
Responsibilities
Conduct comprehensive evaluations of assigned patients.
Utilize clinical reasoning to develop appropriate treatment plans aimed at patient progress towards discharge.
Document medical records accurately and timely, including goal writing and functional assessments.
Provide care based on valid physician orders and insurance approvals.
Supervise and mentor students involved in the therapy program.
Create a personalized professional development plan and pursue relevant continuing education.
Adhere to state practice acts and maintain professional licensure.
Engage in quality improvement initiatives to enhance service delivery.
Required Experience / Certifications / Licensure
Degree in Physical Therapy from an accredited school.
Minimum of 1 year of outpatient clinical experience.
Current CPR certification.
Active, applicable license for the state where service will be provided.
Schedule
This position operates on a standard 5x8 hour shift schedule.
