
LPN Patient Placement Specialist

Job Description
Initiates, promotes, and participates in interdisciplinary collaboration regarding patient placement. Other duties as assigned. Identifies needs and operational opportunities to improve patient access and bed turnaround. Continuously re-evaluates patient placement requests and adjusts staffing and bed management plans accordingly. Supports and promotes excellence in customer and guest relations. Maintains a professional, collaborative, and congenial rapport with internal and external contacts. Demonstrates commitment to professional and personal growth by completing mandatory educational requirements, maintaining unit-based certification, attending staff meetings, and participating in clinical ladder. Supports, promotes, and participates in community service activities. Reports any interruptions of operational flow to the director. Manages patient placement operations within the hospital, ensuring timely admissions and transfers according to hospital policies and procedures. Serves as the single point of contact for all patients placed within the hospital, maintaining communication with referring physicians and transferring hospitals. Evaluates available cardiac beds and develops contingency bed management plans with charge staff to ensure uninterrupted daily operations.
Knowledge, Skills, and Abilities
Must be knowledgeable about medical terminology, levels of care, treatment modalities, and the health care delivery system with demonstrated ability to abstract medical information and interpret patient records.
Requires good interpersonal skills with ability to relate well with administration, physicians, patients, and other hospital employees.
Skills in the use of personal computer and related software systems and applications.
