Aging Care Manager 1 - Area Agency on Aging
Wayne County Area Agency on Aging is seeking an Aging Care Manager who is patient, compassionate, and organized. The role involves working with older adults, their families, and professionals to provide ongoing care management support, including protective services, care transitions, and person-centered counseling.
Key responsibilities include managing a caseload of consumers, assisting in various areas, coordinating services with physicians, community agencies, financial institutions, and other stakeholders on a regular basis.

Qualifications
Three years as an Aging Case Aide 2;
A bachelor's degree with or supplemented by twelve (12) credits in social sciences, behavioral sciences, human services, or a closely related field;
Equivalence of experience and training; candidates are considered qualified 3 months before degree graduation;
Must meet PA residency requirement; preference for candidates residing within Lackawanna, Monroe, Pike, Susquehanna, or Wayne counties;
Ability to perform essential job functions.
Special Requirements
Conditional offer requires drug screening;
Must pass a background investigation, including PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance under the Child Protective Services Law;
