Duties:
Conduct patient evaluations assessing mobility, strength, balance, gait, pain, and functional limitations.
Assess home environment for safety risks and fall hazards.
Develop individualized treatment plans with measurable goals and visit frequency.
Complete required documentation including OASIS and physician orders.
Provide therapeutic exercises, gait and balance training, and functional mobility training.
Assist patients with transfers, stair navigation, and use of assistive devices.
Support daily living activities such as bed mobility and home safety tasks.
Adjust treatment plans based on patient progress and clinical response.
Educate patients and caregivers on home exercise programs, fall prevention, and safety techniques.
Train caregivers on proper mobility and transfer assistance.
Coordinate care with physicians and interdisciplinary team members.
Participate in discharge planning and recommend ongoing care or services.
Requirements:
Master's (MPT) or Doctorate (DPT) in Physical Therapy from an accredited program.
Home Health experience required.
Strong experience with OASIS documentation.
Certification(s):

Active Physical Therapist license (state-specific), MPT or DPT.
Additional Requirements:
Ability to travel daily to patient homes within assigned territory (50 miles radius).
Valid driver's license and reliable transportation.
Comfortable working in home-based care environments.
