Responsibilities
Conduct initial evaluations and ongoing assessments of residents to establish an OT Plan of Care.
Implement therapy interventions to improve activities of daily living, functional mobility, safety with transfers, and independence.
Document progress notes, update goals, and contribute to discharge planning; participate in interdisciplinary team rounds.
Recommend and educate residents and staff on assistive devices, adaptive equipment, and environmental modifications.
Collaborate with nursing staff and therapy colleagues to coordinate care and ensure continuity of services.
Maintain timely, compliant documentation and communicate changes in resident status to the care team.
Requirements
Degree in Occupational Therapy from an ACOTE-accredited program.
Active, unrestricted OT license or eligibility to practice in Texas.
Experience in long-term care or rehabilitation settings preferred.
Strong written and verbal communication skills with proficient documentation.
Ability to work in a day shift and travel to the assignment location; commitment to a 13-week assignment.
