Responsibilities
Perform comprehensive physical therapy evaluations for residents to assess mobility, strength, ROM, balance, endurance, and functional needs
Develop and implement individualized treatment plans to improve mobility, gait, transfers, strength, and activities of daily living
Administer therapeutic interventions (therapeutic exercise, gait training, balance activities, manual therapy, modalities as appropriate) and progress as tolerated
Document evaluation, treatment progress, goals, and outcomes in the designated rehabilitation software (NetHealth Rehab Optima) and coordinate with the interdisciplinary team
Reassess residents regularly, modify plans of care, and communicate progress to physicians, nursing staff, families, and other care team members
Ensure safety, infection control, and compliance with facility policies and regulatory standards
Requirements
Active California Physical Therapy license
Minimum 1 year SNF experience preferred
NetHealth Rehab Optima knowledge preferred
Ability to work the Day shift and commit to 38 hours per week
Willingness to participate in a 13-week travel assignment in Monterey, CA
