Responsibilities
Conduct comprehensive PT evaluations for residents and establish measurable goals for mobility, balance, gait, strength, and activities of daily living.
Design, implement, and monitor individualized treatment plans aligned with resident goals and accepted standards of care.
Provide evidence-based therapeutic interventions, including gait training, therapeutic exercises, balance activities, and functional mobility techniques.
Document assessment findings, treatment plans, progress notes, and discharge summaries in the resident’s medical record; communicate progress with the interdisciplinary team.
Collaborate with nursing, therapy assistants, and other disciplines to coordinate care and support comprehensive resident recovery and safety.
Educate residents and families on home exercise programs, safety strategies, and steps to maintain progress after discharge.
Requirements
Active PT license in the United States (or eligibility for licensure to practice physical therapy)
Ability to work effectively in a skilled nursing facility setting with a geriatric population
Strong communication and documentation skills; reliable collaboration with an interdisciplinary teamWillingness to comply with travel assignment requirements and facility policies
