Summary
Intermittent Patient Aligned Care Team (PACT) Outpatient Registered Nurse executes position responsibilities that demonstrate leadership and creative approaches to management of complex primary care. The nurse provides gender‑specific direct Veteran care and care coordination for Veterans and their care within the Primary Care Department. Manages assigned panels, utilizes registries, and ensures appropriate evaluation and access for patient panels.
Qualifications
English Language Proficiency – required to serve in direct patient care positions.
Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by ACEN or CCNE, or attending a master’s level bridge program with equivalent coursework.
Upon achievement of a state license, temporary appointment subject to conversion to permanent upon completion of the bridge program.
For foreign graduates, possession of a current, full, active, and unrestricted registration from a U.S. State, Territory, or Commonwealth; formal equivalency validation required for degrees higher than associate.
Graduate Nurse Technician (GNT) Exception – may be appointed at entry level while pursuing licensure (up to two years, extended case‑by‑case).
Preferred Experience – seasonal vaccine coordinator with comprehensive knowledge of vaccine screening and administration, consulting on policies and procedures, collaborating with primary care leadership, and implementing clinical reminders.
All current VHA 0610 series staff performing these duties as of 1/29/2024 are grandfathered as meeting qualification requirements.
Grade Determinations – specific scope, education, and performance criteria for each grade and level are detailed in the online assessment.
Physical Requirements: moderate lifting and carrying (15‑44 lbs), moderate pulling and pushing, reaching above shoulder, use of fingers, both hands required, walking 4 hours, standing 4 hours, kneeling 0.5 hour, rapid mental and muscular coordination, depth perception, color distinction, hearing (aid permitted), mental and emotional stability.
Environmental Factors: working closely with others.
Duties
Provide health promotion, health education, and coaching on wellness, disease prevention, and chronic care management.
Collaborate in the development, implementation, evaluation, and revision of policies, procedures, and guidelines.
Demonstrate performance and leadership to improve care within the Primary Care Department and facility.
Provide feedback to peers for improvement.
Support colleagues and other nurses through primary care knowledge sharing.
Foster a safe and supportive environment conducive to professional development.
Evaluate outpatient outcomes with evidence‑based decisions and practice changes for individuals, groups, and populations.
Document PACT outcomes at program or service level; outcomes should be broad, complex, and demonstrable at any organizational level.
Assume responsibility for all elements of the nursing process when providing or supervising direct patient care.
Assess, plan, implement, and evaluate care based on age‑specific components.
Coordinate care focused on patient education, self‑management, and customer satisfaction throughout the continuum of care.
Administer medications and procedures per established policies and guidelines.
Influence care outcomes by collaborating with interdisciplinary team members.
Serve as seasonal vaccine coordinator with leadership in vaccine administration and policy.
