Job Summary
Integrated Behavioral Health Therapist is responsible for independently providing assessments, treatment/care planning and psychosocial therapy within a primary care clinic setting for a diverse population of preventative, acute care and chronic disease management patients.
Essential Duties and Responsibilities
Implements mental health assessments and utilizes the DSM-5 to provide an accurate diagnosis/differential diagnoses.
Provides person‑centered treatment planning based on the assessments.
Employs various therapeutic interventions designed to address identified concerns and support needs.
Documents individual and group sessions and employs client treatment plan as a tool to increase client engagement and ownership.
Documents communication with primary care clinic providers and outside support resources.
Collaborates and maintains positive relationships with all primary care team members and outside resources associated with the primary care clinic to facilitate quality patient care.
Meets facility productivity and documentation standards.
Participates in quarterly care team meetings and other meetings as requested by clinic leadership.
Maintains reliable attendance.
Contributes to the success of the organization by meeting organizational competency expectations and core values (respect, integrity, stewardship, excellence, collaboration and kindness), continuously learning, and by performing other duties as needed or assigned.
Practices the proper fire, safety, and disaster procedures as set forth by SMC.
Participates in all safety, security and infection control programs that are mandatory as well as those required and provided by the department.
Upholds confidentiality and HIPAA compliance at all times and reports violations to the Privacy Officer.
Understands and complies with the requirements of the Saunders Medical Center’s Compliance Program, including the Code of Conduct, the Compliance Policy, all Saunders Medical Center‑wide policies for compliance, and compliance plans affecting specific duties and responsibilities.
Participates in all education and training programs regarding compliance as required by Saunders Medical Center policy, licensure and as requested by the Supervisor.
Demonstrates personal traits of a high‑level commitment, motivation and energy; team orientation; professionalism; trust; personal honesty and integrity; and an ability to work independently.

Qualifications
Master’s degree from an accredited institution in Social Work, Counseling, Marriage & Family Therapy, or a related field. Two years of experience providing direct therapy is preferred.
Current licensure as a Licensed Clinical Social Worker (PCMSW, CMSW) or Licensed Mental Health Practitioner (PLMPH, LMHP, LIMHP). Provisional status may be considered.
Language skills: Ability to read, write and follow written and oral instructions, and to communicate effectively with personnel, patients, visitors and the general public.
Mathematical skills: Ability to calculate at the level necessary to perform essential duties.
Reasoning ability: Ability to make independent decisions when circumstances warrant and to interpret a variety of instructions furnished in oral, written, diagrammatic or scheduled form.
Physical Demands
The role requires frequent sitting, long periods of document review, occasional standing, reaching with arms, lifting, pushing or pulling of objects up to a specified weight, and occasional walking. Reasonable accommodations may be provided to meet these demands.
Work Environment
Work is conducted in an office and patient care areas. Attendance is essential and required for a successful working environment.
